out the information below so we can collect a little information on you, as well
as the business you are referring. Once the referred business account activates
for merchant processing we will issue you a $100 credit on your next invoice.

** To take advantage of this program you must be a merchant customer of Fifth Third Processing Solutions at the time of the referral and activation of the referred business. $100 credit will be issued to your Services Invoice after activation of the referred business. You may not
refer business in which you are an owner or officer or any business that is a member of a franchise or association with special pricing. A complete, accurate referral
form must be received prior to the referred merchant signing a contract with Fifth Third Processing Solutions. Referral payments will only be made for new merchants,
not for existing customers or customers which Fifth Third Processing Solutions may receive as a result its normal sales efforts or through third party relationships
without the use of information provided by you. Fifth Third Processing Solutions reserves the right to change or cancel this program at any time. $100 credit will
be issued to your Services Invoice after activation of the referred business.
